Announcement Regarding the COVID-19 Vaccine

SOSC Vaccination Disclosure Form

Effective May 23, 2022, all registered SOSC participants (athletes, coaches, volunteers, and employees) are required to disclose their COVID-19 vaccination status via this form in order to participate in future sports programming, training, and events. The data collected will be used to inform SOSC on current COVID-19 vaccination trends within the organization and will help us to determine appropriate safety measures and COVID-19 protocols that will be used in future sports programming, training, and events.

Fully vaccinated for SOSC purposes is defined as the following:

Two (2) doses of Pfizer or Moderna plus a booster
OR
One (1) dose of Johnson & Johnson plus a booster
OR
2 doses of Pfizer or Moderna or 1 dose of Johnson & Johnson, but not yet eligible for a booster dose

For the specific purposes of SOSC’s data collection, a decision to not disclose your COVID-19 vaccination status will be treated and tabulated with those persons who identified as being not vaccinated. If you are not fully vaccinated, but plan to become fully vaccinated in the future, please complete this form after you have become fully vaccinated.

All vaccine information will be kept confidential with very limited access.

Spring Sports Festival

Those participants that are currently registered for the Spring Sports Festival at CSULB on June 18, 2022 are required to be fully vaccinated and must provide valid proof of vaccination before June 10, 2022 at 9 am (PST) in order to maintain eligibility for this event. Please upload a JPG or PDF copy of your COVID-19 vaccination card or other proof of COVID-19 vaccination when submitting the online form. With the recent notification that CSULB has dropped their vaccine requirement for our use of their campus, along with SOSC being in the latter stages of planning for an event being held in less than a month, SOSC’s Board Executive Committee has agreed that it would be too disruptive to lift the requirement after so many have already signed up. It is our understanding that many who registered signed up because a vaccine requirement was in place. Thank you for your understanding, support and patience as we focus on athlete safety, health, fun and teamwork in a very fluid environment.
